All CIDER 10 Participants:

All participants seeking reimbursement who have not been reimbursed by the UC Berkeley within the last two years will need to fill out the following form and fax it to +1 (510) 642-8604.

  • Berkeley Business Request Form
  • Visa holders should go to the website below to determine what additional documentation you will need to provide for reimbursement. Links to some of the common required documents are also below.

  • Quick Guide - Foreign Visas Travel Reimbursement

  • University of California Certificate of Foreign Status for Federal Tax Withholding (UC W-8BEN)

  • UC Certificate of Academic Activity (for B-1, B-2, WB, and WT visa holders)


  • Below are the travel reimbursement forms for CIDER. You should click on the form that best describes your geographical location. Once you fill out the form, please email it to resreimburse@berkeley.edu or fax it to TBD. Please note that you will need to mail your original receipts for expenses over $74.99 and for expenses such as rental cars, gasoline.
